This form has several sections. The first part appears below:
You must bear in mind that the information provided in Fig. 1 must match the information that appears on your electricity, water or telephone bill that you will send to TD Ameritrade. On the next page of the form you will fill it out as follows:
As you can see in Fig. 2, in the occupation category I wrote D41 and in the industry category I wrote H11. These codes are given on the last page of the application:
In Fig. 3 we present the list of codes, as you can see, D41 corresponds to doctor and H11 to healthcare and medical services. This is an example, in your case you have to consider the codes that identify your occupation and the industry in which you work. Once this is done, you have to write down your income, as shown below:
You must bear in mind that the data given in Fig. 4 depend on your personal income and its source. However, whether your income is much higher or lower, I recommend you to choose the data given in Fig. 4 to avoid additional questions in the future. In case you don't have the B1/B2 visa, choose "No" and leave it blank.
- Account co-owner information: Do not write anything, unless you want to create an account for two people.
- Affiliations: Do not write anything.
- Investment objectives: Mark as follows:
Fig. 5 shows that you are a moderate investor who expects to obtain gains in the medium term. This information is not relevant so you can choose whatever you like.
- Margin privileges: Do not write anything. This will allow the margin-trade option to remain active in your personal account.
- Funding your account: Choose "a wire transfer to be initiated after account opening."
- Cash Sweep Vehicle: Do not type anything.
- Trade confirmations: Choose "electronic monthly / electronic." It is a free service.
- Verbal password: Write a word that you will use as a password every time you make inquiries by phone.
- Offer code: Do not write anything.
- Trusted contact: Write the details of a trusted person (e.g., a first-degree relative). If there are problems with your account and you cannot be contacted directly, TD Ameritrade will contact this person.
- Account Agreement: All you have to do is sign and write the date.
- Options objectives: Mark as follows:
This gives you access to "options," which is a high risk investment method. If you are not familiar with it you may never use it, but having it activated will be very useful in the future if your knowledge increases progressively. Keep in mind that this is free, that is, you will not be charged if you activate or deactivate this option. To deactivate it, click on the upper box in Fig. 6, where it says "check this box to decline options privileges," and in point 14 leave everything blank.
- Options account agreement: Just sign and write the date.
